Part I — Taxpayer represented

This is the person, estate, or entity you are filing on behalf of. Pick the identifier that fits: an SSN for a living person, an EIN for a business / trust / estate, or the decedent's SSN if you are acting for a deceased individual.

Fiduciary (the person or entity you are)

Your identifying info as the fiduciary. You can act in your own name or under a business name (for example, a trust company).
